Our facilities are open Monday through Friday 7:00am to 7:00pm and Saturday 8:00am to 12:00 noon. We are closed Sunday.

Before You Arrive

To prepare for your appointment, please start by filling out the appropriate forms below prior to your appointment.

Getting Started

Depending on your circumstances, a typical session with our physical therapists will be approximately 45 minutes. Here are a few important things to get you started. We encourage you to complete registration forms prior to your visit.

Please dress comfortably for your appointments. You will be required to complete the new patient paperwork, which can be filled out prior to your initial appointment. You should arrive approximately 15 minutes early to be ready for your appointment with your therapist.

Upon arrival we ask that you check in with the front desk.

If you are running late, please call and let us know and we can try to accommodate your schedule.

Cancellation policy requires that you provide us with a 24 hour advance notice if you need to cancel or reschedule; otherwise you may be assessed a cancellation fee.

ProCare Therapy Services’ policy is to take safety measures in protecting our patient’s identity. All new patients are required to provide a valid photo identification (Driver’s License, State ID, Passport, School ID), along with their health insurance cards.

After your consultation, you can also expect a thorough, interactive evaluation from a Licensed Therapist in order to establish a customized treatment plan that will assist you in meeting your functional goals. During this evaluation you will have the opportunity to discuss your condition, help set goals for your therapy, and gain a good understanding of what you may expect from attending therapy visits including prognosis, treatment interventions and approximately how long it should take. At the end of the first treatment you should receive a copy of your home exercise followed by a quick check out process. Initial visit takes about 45-60 minutes.

On all follow up visits, you simply check in at the front desk and expect to wait no longer than 5 minutes for a qualified therapist to assist you to the treatment room. From here you will continue your treatment progression. Follow up visits typically last 45 minutes.
